I've got a broken right rear spring and am going to replace it (1990 525i).
Should I replace the left rear at the same time? It's all stock suspension and I'm not too interested in lowering it but might consider a different spring that works with stock shock to improve handling (if such a thing exists).
Anything else that should be replaced while the spring is out? Pads, etc. And would that be cause for replacing both sides?

Well, if you're going to be putting a used spring in, I think it would be fine to just swap one. I doubt you'll see a huge difference in ride height between the two.
You could always just buy a pair and measure the difference between the new and the old. I'm sure there are dozens of people on this board that have swapped out springs and have an old set they'd let go for $20 or so.
